Summary: Cockpit voice recordings reviewed by the NTSB say a pilot on an Amazon cargo flight repeatedly warned his co-pilot they were going too fast as they approached and after touchdown. The plane touched down awkwardly, briefly attempted to take off, then overran about 396 metres past the Miami runway, hitting a van with five workers and an SUV before catching fire.
